This information should be e-mailed to Ms. Chrystal Smith. She will then respond within 24 hours letting you know that your aaplication has been received.
A meeting registration fee of $100 will be required. This fee will include the banquet meal on Friday evening as well as be used to cover the costs associated with refreshments during the welcome reception, poster session, and at coffee breaks. The registration fee will be collected at the time of the meeting.
